Adult Services Coordinator (ASC)

The Moultrie County Health Department is seeking a part-time Adult Services Coordinator (24 hours/week) to:

  • Assess needs, make referrals, and ensure engagement for adult caregivers and MCHD clients.
  • Comply with grant funding and program mandates.
  • Expand professional partnerships and service referral systems.
  • Conduct community education and outreach activities, including public presentations.

Qualifications include:

    • B.A. or B.S. in community health, social work, or a related field.
    • At least two years of professional experience working with adults in community health, public health, adult education, memory care, or adult social services.
      • Ability to work with diverse groups of people required.
      • Experience working with older adults preferred.
      • Excellent communication, organizational, and decision-making skills.
